Photo/Video Release & Permission of a Minor

I hereby consent that any photographs and videos taken of my child may be used by Maumee UMC and its assigns and successor for any internal publications including but not limited to, brochures, newsletters, and posters. I also here by consent that any photographs or videos taken of my child may be used by Maumee UMC for external publications, including, but not limited to television production, social media, and the church website. Furthermore, I consent that such photographs shall be property of Maumee UMC, which has the right to duplicate, and make use as deemed necessary, in accordance with this policy, free and clear of any claim whatsoever on my part. - I understand that photographs and videos appearing on the church website and/or social media will not include names of children. - I release them from any expectation of confidentiality for the undersigned minor child and attest that I am the parent or legal guardian of the child listed above.

I DO give Maumee UMC my consent to use my child's photo or video. I DO NOT give Maumee UMC permission to use photographs or videos of my child in any capacity.
